体重指数及血脂指标与幽门螺杆菌感染的关系[

  • 目的  探讨体重指数及血脂指标与幽门螺杆菌(Hp)感染的关系。方法  选取2023年1—4月于河北省人民医院进行健康体检者2 000例为研究对象。所有体检者均行13C-尿素呼气试验检查,统计体检者Hp阳性情况。根据Hp测定结果,将体检者分为Hp阳性组和Hp阴性组。比较2组体重指数和血脂指标水平。分析Hp阳性与体重指数和血脂指标的相关性以及Hp阳性的危险因素。结果  2 000例健康体检者中,Hp阳性747例(37.4%,Hp阳性组),Hp阴性1 253例(62.6%,Hp阴性组)。Hp阳性组体重指数正常比例低于Hp阴性组,超重、肥胖比例均高于Hp阴性组(均P<0.001)。Hp阳性组高密度脂蛋白胆固醇(HDL-C)下降及低密度脂蛋白胆固醇(LDL-C)、三酰甘油、总胆固醇升高比例均高于Hp阴性组(均P<0.001)。Pearson相关性分析结果显示,Hp阳性与体重指数、LDL-C、三酰甘油和总胆固醇呈线性正相关(r=0.582、0.547、0.618、0.534,均P<0.001),与HDL-C呈线性负相关(r=-0.651,P<0.001)。多因素Logistic回归分析结果显示,体重指数≥24 kg/m2、HDL-C下降、LDL-C升高、三酰甘油升高和总胆固醇升高为Hp阳性的独立危险因素(均P<0.05)。结论  体重指数升高和血脂异常与Hp感染密切相关,且为影响Hp感染独立危险因素。

  • Objective  To explore the relationship between body mass index (BMI), blood lipid indicators and Helicobacter pylori (Hp) infection. Methods  Totally 2 000 healthy individuals who underwent physical examinations in Hebei General Hospital from January to April 2023 were selected as the research subjects. All individuals underwent 13C urea breath test and were counted for Hp positivity, and they were divided into Hp positive group and Hp negative group. BMI and blood lipid indicators were compared between the two groups. The correlation of Hp positive with BMI and lipid indicators, and the risk factors of Hp positive were analyzed. Results  Among the 2 000 healthy individuals undergoing physical examination, 747 cases (37.4%, Hp positive group) were Hp positive, and 1 253 cases (62.6%, Hp negative group) were Hp negative. The rate of normal BMI in Hp positive group was lower than that in Hp negative group, and rates of overweight and obesity in Hp positive group were higher than those in Hp negative group (all P<0.001). The rates of high-density lipoprotein cholesterol (HDL-C) decreased and low-density lipoprotein cholesterol (LDL-C), triacylglycerol and total cholesterol increased in Hp positive group were higher than those in Hp negative group (all P<0.001). Pearson correlation analysis showed that Hp positive was positively correlated with BMI, LDL-C, triacylglycerol and total cholesterol (r=0.582, 0.547, 0.618, 0.534, all P<0.001), and was negatively correlated with HDL-C(r=-0.651,P<0.001). Multivariate Logistic regression analysis showed that BMI≥24 kg/m2, HDL-C decreased and LDL-C, triacylglycerol and total cholesterol increased were independent risk factors for Hp positive. Conclusion  Elevated BMI and dyslipidemia are closely related to Hp infection, and are independent risk factors for Hp infection.
