目的 研究肿瘤恶性增殖及侵袭相关基因在乳腺癌组织中表达水平与前哨淋巴结(SLN)阳性数的相关性。方法 选取2018年1月至2019年12月西北民族大学第一附属医院收治的乳腺癌患者80例,根据SLN病理诊断结果,将患者分为SLN阳性组和SLN阴性组,各40例。检测2组乳腺癌组织及SLN阳性组患者癌旁组织中肿瘤恶性增殖及侵袭相关基因mRNA表达量,并分析其与SLN阳性数的相关性。结果 SLN阳性组癌组织中张力蛋白mRNA表达量低于SLN阴性组和癌旁组织,磷脂酰肌醇-3-激酶(PI3K)、细胞周期蛋白D1及蛋白激酶B(AKT) mRNA表达量均高于SLN阴性组和癌旁组织[(0.48±0.07)比(1.39±0.22)、(0.81±0.13),(1.82±0.44)比(0.92±0.14)、(1.01±0.19),(1.72±0.34)比(0.12±0.10)、(0.67±0.21),(2.04±0.37)比(0.81±0.19)、(1.04±0.23)](均P<0.05)。SLN阳性组癌组织中E钙黏蛋白mRNA表达量低于SLN阴性组和癌旁组织,N钙黏蛋白、β连环蛋白及转化生长因子β(TGF-β)mRNA表达量高于SLN阴性组和癌旁组织(均P<0.05)。Pearson相关性分析显示,细胞周期蛋白D1、PI3K、AKT、N钙黏蛋白、β连环蛋白及TGF-β mRNA表达量与SLN阳性数呈正相关,张力蛋白、E钙黏蛋白mRNA表达量与SLN阳性数呈负相关(均P<0.05)。多因素Logistic回归分析结果显示,张力蛋白、PI3K及AKT是SLN阳性数的独立预测因子(均P<0.05)。结论 肿瘤恶性增殖及侵袭相关基因mRNA表达量与SLN阳性数相关,且张力蛋白、PI3K以及AKT是SLN阳性数的独立预测因素。
Objective To investigate the correlation between expressions of tumor malignant proliferation and invasion-related genes in breast cancer tissues and positive number of sentinel lymph nodes(SLN). Methods From January 2018 to December 2019, 80 patients with breast cancer who admitted to the First Affiliated Hospital of Northwest Minzu University were enrolled. According to SLN pathological diagnosis results, they were divided into the SLN-positive group and SLN-negative group, with 40 cases in each group. The expressions of tumor malignant proliferation and invasion-related genes mRNA in breast cancer tissues of both groups and in para-cancerous tissue of SLN-positive group were detected. The correlation between the expressions and positive number of SLN were analyzed. Results The expression of phosphoatase and tensin homolog(PTEN) mRNA in cancerous tissue of SLN-positive group was lower than that in SLN-negative group and para-cancerous tissue, and the expressions of phosphatidylinositol 3-kinase(PI3K), cyclin D1 and protein kinase B(AKT) mRNA in cancerous tissue of SLN-positive group were higher than those in SLN-negative group and para-cancerous tissue [(0.48±0.07) vs (1.39±0.22),(0.81±0.13);(1.82±0.44) vs (0.92±0.14),(1.01±0.19);(1.72±0.34) vs (0.12±0.10),(0.67±0.21);(2.04±0.37) vs (0.81±0.19),(1.04±0.23)](all P<0.05). The expression of E-cadherin mRNA in cancerous tissue of SLN-positive group was lower than that in SLN-negative group and para-cancerous tissue, and the expressions of N-cadherin, β-catenin and transforming growth factor-β(TGF-β) mRNA in cancerous tissue of SLN-positive group were higher than those in SLN-positive group and para-cancerous tissue (all P<0.05). Pearson correlated analysis showed that the expressions of cyclin D1, PI3K, AKT, N-cadherin, β-catenin and TGF-β mRNA were positively correlated with SLN positive number, and the expressions of PTEN and E-cadherin were negatively correlated with SLN positive number (all P<0.05). Multivariate Logistic regression analysis showed that PTEN, PI3K and AKT were independent predictors of SLN positive number (all P<0.05). Conclusion s The expressions of tumor malignant proliferation and invasion-related genes are related to the SLN positive number. PTEN, PI3K and AKT are independent predictors of SLN positive number.
