目的 探讨维生素D干预对单纯性肥胖合并维生素D缺乏儿童糖脂代谢的影响。方法 选取2019年1—8月在湖南省儿童医院儿童保健所门诊就诊的单纯性肥胖合并维生素D缺乏儿童84例,应用随机数字表法分为观察组和对照组,各42例。对照组接受营养师提供的营养指导及饮食行为矫正干预6个月,观察组在对照组干预基础上口服维生素D 6个月。比较2组干预前后体重指数、血清25-羟维生素D[25-(OH)D]和糖脂代谢指标水平。结果 干预后,2组体重指数和血清三酰甘油、总胆固醇、低密度脂蛋白胆固醇、空腹血糖和空腹胰岛素水平均低于干预前,且观察组体重指数和血清三酰甘油、总胆固醇、空腹胰岛素水平均低于对照组[(24±4)kg/m2比(26±3)kg/m2,(0.8±0.3)mmol/L比(1.1±0.4)mmol/L,(3.8±0.5)mmol/L比(4.1±0.6)mmol/L,(8.4±2.3)mIU/L比(11.0±1.8)mIU/L];血清25-(OH)D和高密度脂蛋白胆固醇水平均高于干预前,且观察组血清25-(OH)D水平高于对照组[(70±10)nmol/L比(51±9)nmol/L](均P<0.05)。结论 在肥胖干预基础上补充维生素D有助于改善单纯性肥胖合并维生素D缺乏儿童糖脂代谢紊乱。
Objective To investigate the effect of vitamin D intervention on glycolipid metabolism in children with simple obesity and vitamin D deficiency. Methods Totally 84 children with simple obesity and vitamin D deficiency were selected from January to August 2019 in the outpatient of Children′s Healthcare Centre, Hunan Children′s Hospital. They were randomly divided into observation group and control group, with 42 cases in each group. The control group received nutritional guidance and dietary behavior provided by nutritionists for 6 months intervention, while the observation group received vitamin D oral intervention on the basis of the control group for 6 months. Body mass index, serum 25-hydroxy vitamin D [25-(OH)D] and glycolipid metabolism indexes levels were compared between the two groups before and after intervention. Results After intervention, the body mass index, serum triglyceride, total cholesterol, low-density lipoprotein cholesterol, fasting blood glucose and fasting insulin levels in the two groups were lower than those before intervention, and the body mass index, serum triglyceride, total cholesterol and fasting insulin levels in the observation group were lower than those in the control group [(24±4)kg/m2 vs (26±3)kg/m2, (0.8±0.3)mmol/L vs (1.1±0.4)mmol/L, (3.8±0.5)mmol/L vs (4.1±0.6)mmol/L, (8.4±2.3)mIU/L vs (11.0±1.8)mIU/L]; serum 25-(OH)D and high density lipoprotein cholesterol levels in both groups were higher than those before intervention, and the serum 25-(OH)D level in the observation group was higher than that in the control group [(70±10)nmol/L vs (51±9)nmol/L] (all P<0.05). Conclusion Vitamin D intervention can improve the glucose and lipid metabolism in children with simple obesity and vitamin D deficiency.
