头孢克洛配合芒硝外敷治疗急性乳腺炎的临床效果及对白细胞介素和细胞间黏附分子1蛋白表达水平的影响

• 【摘要】目的    观察头孢克洛配合芒硝外敷治疗急性乳腺炎的临床效果及对白细胞介素（IL）和细胞间黏附分子1（ICAM-1）蛋白表达水平的影响。方法    选取2015年7月至2017年12月西安交通大学第二附属医院诊治的180例急性乳腺炎患者为研究对象。完全随机分为对照组和观察组，各90例。对照组患者口服头孢克洛干混悬剂治疗，观察组患者在对照组基础上配合芒硝外敷治疗，2组均治疗14 d。分析 2组受试者的临床治疗效果及治疗前后IL和ICAM-1蛋白表达水平的变化。结果    观察组总有效率高于对照组［95.6%（86/90）比78.9%（71/90）］，差异有统计学意义（χ2=2.856，P<0.001）。治疗后，2组IL-1β、IL-2、IL-6、ICAM-1、血管细胞黏附分子1水平均较治疗前降低，且观察组低于对照组［（35±5）ng/L比（40±6）ng/L、（34±5）ng/L比（40±6）ng/L、（40±6）ng/L比（44±6）ng/L、（0.39±0.07）比（0.44±0.06）、（0.35±0.08）比（0.41±0.08）］，差异均有统计学意义（均P<0.05）。结论    头孢克洛配合芒硝外敷能提高急性乳腺炎的治疗效果，且对IL和ICAM-1蛋白表达水平有明显的恢复作用。

• 【Abstract】Objective    To observe the clinical effect of cefaclor combined with external use of mirabilite on acute mastitis and the expression levels of interleukin(IL) and intercellular adhesion molecule-1（ICAM-1）. Methods    A total of 180 patients with acute mastitis admitted to the Second Affiliated Hospital of Xi′an Jiaotong University from July 2015 to December 2017 were randomly divided into control group and observation group, with 90 cases in each group. The control group took cefaclor dry suspension while the observation group was treated with cefaclor and external use of mirabilite for 14 days. Clinical efficacy was observed and expression levels of IL and ICAM-1 were detected before and after treatment. Results    The total effective rate in observation group was significantly higher than that in control group［95.6%（86/90） vs 78.9%（71/90）］（χ2=2.856， P<0.001）. After treatment, serum levels of IL-1β, IL-2, IL-6, ICAM-1 and vascular cell adhesion molecule-1 were significantly lower compared to those before treatment; the levels in observation group were significantly lower compared to those in control group［（35±5）ng/L vs （40±6）ng/L, （34±5）ng/L vs （40±6）ng/L, （40±6）ng/L vs （44±6）ng/L, （0.39±0.07） vs （0.44±0.06）, （0.35±0.08） vs （0.41±0.08）］(all P<0.05). Conclusion    Cefaclor combined with external use of mirabilite can improve the therapeutic effect on acute mastitis and promote the recovery of serum levels of IL and ICAM-1.
