【摘要】目的 分析Sysmex XN-9000型血细胞分析仪与牛鲍手工法在体液标本检测中的应用效果及对肿瘤细胞筛查的敏感度。方法 收集2016年12月至2018年3月上海长海医院疑似恶性肿瘤100例患者的腹腔积液、脑脊液、胸腔积液。采用Sysmex XN-9000型血细胞分析仪对体液标本进行分析,记录高荧光强度有核细胞计数(HF-BF#)、高荧光强度有核细胞比例(HF-BF%),采用牛鲍手工法对体液标本进行分析,记录体液分析模式的红细胞计数(RBC-BF)及有核细胞计数。以病理学检测结果为金标准,分析各指标筛查肿瘤细胞的价值。结果 本研究中HF-BF#中位数为7×106/L,HF-BF%中位数为2%,RBC-BF中位数为680×106/L,有核细胞计数中位数为38×106/L。HF-BF#、HF-BF%、RBC-BF及有核细胞计数筛查肿瘤细胞的敏感度、特异度、阳性预测值、阴性预测值及准确率均高于80.00%,且4项指标筛查诊断的曲线下面积均高于0.85。结论 采用Sysmex XN-9000型血细胞分析仪与牛鲍手工法对体液标本中肿瘤细胞进行筛查的应用效果均较好,且均具有较高的敏感度。
【Abstract】Objective To examine the application of Sysmex XN-9000 and Neubauer′s method in body fluid specimen detection, and to analyze the sensitivity in tumor cell screening. Methods From December 2016 to March 2018, 100 cases of ascites, cerebrospinal fluid and pleural effusion of suspected malignant tumors were collected from Shanghai Changhai Hospital. Body fluid samples were analyzed by Sysmex XN-9000 to obtain high fluorescence intensity nucleated cell count(HF-BF#) and high fluorescence intensity nucleated cell ratio(HF-BF%); red blood cell count of humoral analysis model(RBC-BF) and nucleated cell count were recorded by Neubauer′s method. Pathological examination was used as the standard to evaluate the values of each index in screening tumor cells. Results The median of HF-BF was 7×106/L; the median of HF-BF% was 2%; the median of RBC-BF was 680×106/L; the median of nucleated cell count was 38×106/L. Sensitivities, specificities, positive predictive values, negative predictive values and accuracies of HF-BF#, HF-BF%, RBC-BF and nucleated cell count in screening tumor cells were all higher than 80.00% and the areas under curve were all higher than 0.85. Conclusion Sysmex XN-9000 and Neubauer′s method have high sensitivity in screening of tumor cells.
