干燥综合征患者唇腺浆细胞分布特点及相关因素的研究

  • 【摘要】目的    研究干燥综合征(SS)患者唇腺浆细胞分布特点及相关因素。方法    收集2013年7月至2016年12月52例在北京大学第一医院风湿免疫科行唇腺活检患者(SS患者28例,非SS患者22例)的临床资料,记录患者抗核抗体谱和免疫球蛋白(Ig)水平,取其留存石蜡包埋唇腺组织再次切片进行CD138免疫组织化学染色,计数CD138+浆细胞数量。比较SS患者与非SS患者浆细胞数量差别,分析SS患者浆细胞数量与Ig水平的相关性。结果    SS患者与非SS患者唇腺的间质中均有散在浆细胞,并有数个浆细胞聚集的现象,而SS患者唇腺间质浆细胞数量明显多于非SS患者[(1 263±1 051)个/mm2比(562±360)个/mm2],差异有统计学意义(P=0.002)。SS患者唇腺中浆细胞在淋巴细胞浸润灶内的分布有很大差异,由零星少量浆细胞,到较多浆细胞并有浆细胞聚集现象,再到大量浆细胞聚集成灶。SS唇腺浆细胞聚集灶常出现在大的淋巴细胞聚集灶内及其周边。SS患者唇腺间质内和淋巴细胞灶内的CD138+浆细胞总数与血清IgG和IgA水平均无相关性(r=0.22、P=0.26,r=-0.14、P=0.50)。结论    SS患者唇腺组织中浆细胞明显增多,可以出现浆细胞聚集灶。而SS浆细胞聚集灶的形成与唇腺大的淋巴细胞浸润灶相关。

  • 【Abstract】Objective    To investigate the distribution characteristics of plasma cells in labial glands of Sjgren′s syndrome(SS). Methods    From July 2013 to December 2016, clinical data of 28 SS patients and 24 non-SS patients who had labial gland biopsy in Department of Rheumatology and Immunology, Peking University First Hospital were reviewed, including the records of anti-nuclear antibody and immunoglobulin(Ig). Parafin section of labial glands was tested by immunohistochemistry and the number of CD138+ plasma cells was counted. Relation between CD138+ plasma cell number and Ig level was analyzed. Results    CD138+ plasma cells presented in interstitial tissue of labial glands in both SS and non-SS patients; the number of CD138+ plasma cells in SS patients was significantly higher than that in non-SS patients[(1 263±1 051) cells/mm2 vs (562±360)cells/mm2](P=0.002). In SS patients, the distribution of plasma cells in infiltrating lymphocytic foci presented significant differences among different individuals; some were scattering; some cells aggregated; some were plasma cell aggregation foci. Most plasma cell aggregation foci were near the large lymphocyte foci. The total number of CD138+ plasma cells in interstitial tissue and lymphocyte foci showed no significant correlation with serum levels of IgG and IgA (r=0.22, P=0.26; r=-0.14, P=0.50). Conclusion    The number of plasma cells increases in labial glands in SS patients; some patients have plasma cell aggregation foci, which may be related to the large lymphocytic infiltrating foci.
