阻塞性睡眠呼吸暂停低通气综合征(OSAHS)是一种以反复上呼吸道阻塞导致间歇性低氧、高碳酸血症和睡眠结构紊乱为主的睡眠障碍,并与心脑血管疾病和代谢性疾病相关,如高血压病、冠心病(冠状动脉粥样硬化性心脏病)、糖尿病等。近年来,相关研究发现脂联素是一种具有抗炎和抗动脉粥样硬化作用的脂肪因子。众多学者关注到脂联素在OSAHS的发生发展中具有潜在的作用。研究发现,OSAHS患者的血清脂联素水平通常较低,且这种降低与疾病的严重程度和肥胖相关。持续气道正压通气治疗可以在一定程度上提高OSAHS患者的血清脂联素水平,改善炎症状态,提示脂联素在OSAHS的发病机制中具有重要作用。此外相关研究揭示,脂联素基因多态性可能影响其表达和功能,从而影响OSAHS的发生发展。本文将综述脂联素在OSAHS中的作用及与其基因遗传相关性的最新研究进展,以期为OSAHS的诊断和治疗提供新的思路和潜在靶点。
Obstructive sleep apnea hypopnea syndrome (OSAHS) is a sleep disorder characterized by intermittent hypoxia, hypercapnia and sleep structure disorder caused by repeated upper airway obstruction, and is associated with cardio-cerebrovascular diseases and metabolic diseases, such as hypertension, coronary heart disease, diabetes, etc. In recent years, related studies have found that adiponectin is an adipokine with anti-inflammatory and anti-atherosclerotic effects. Many scholars have paid attention to the potential role of adiponectin in the occurrence and development of OSAHS. It has been found that serum adiponectin levels are generally lower in patients with OSAHS, and this reduction correlates with disease severity and obesity. Continuous positive airway pressure treatment can increase serum adiponectin level and improve inflammatory state in patients with OSAHS to a certain extent, suggesting that adiponectin plays an important role in the pathogenesis of OSAHS. In addition, related studies have revealed that adiponectin gene polymorphism may affect its expression and function, thereby affecting the occurrence and development of OSAHS. This article reviews the latest research progress on the role of adiponectin in OSAHS and its genetic correlation, in order to provide new ideas and potential targets for the diagnosis and treatment of OSAHS.
