加味消瘰丸联合耳穴压豆治疗肉芽肿性乳腺炎的临床研究

• 摘要：

• 目的  探讨加味消瘰丸联合耳穴压豆对肉芽肿性乳腺炎（GM）患者的临床疗效。方法  选取2021年6月至2023年3月汕头大学医学院第二附属医院收治的热毒痰瘀型GM患者80例，按照随机数字表法分为观察组和对照组，各40例。对照组予以常规西医治疗，观察组在对照组基础上予以加味消瘰丸联合耳穴压豆治疗，2组均治疗6个月。比较2组患者临床总有效率、临床治愈时间、肿块体积缩小差值、视觉模拟量表（VAS）评分、术后Harris标准评价结果、复发率及药物不良反应发生率。结果  观察组临床总有效率高于对照组［90.0%(36/40)比70.0%(28/40)］（P=0.025）。观察组患者临床治愈时间短于对照组［（4.2±1.1）个月比（5.0±1.2）个月］,肿块体积缩小差值大于对照组［（2.9±0.6）cm3比（2.2±0.8）cm3］（均P＜0.05）。治疗后，观察组患者VAS评分低于对照组（P＜0.05），术后乳房外形Harris标准评价优于对照组（P=0.039）。观察组患者治疗6个月内GM复发率和药物不良反应发生率均低于对照组（均P＜0.05）。结论  加味消瘰丸联合耳穴压豆可显著改善GM患者临床症状，提高临床总有效率，并减少复发和不良反应发生的风险。

• Objective To explore the clinical efficacy of Jiawei Xiaoluo pill combined with ear point pressing beans on patients with granulomatous mastitis (GM). Methods Totally 80 patients with heat toxicity phlegm stasis type GM admitted to the Second Affiliated Hospital of Shantou University Medical College from June 2021 to March 2023 were enrolled. Patients were divided into observation group and control group according to the random number table method, with 40 cases in each group. The control group was treated with conventional western medicine, while the observation group was treated with Jiawei Xiaoluo pill combined with ear point pressing beans based on the control group, both groups were treated for 6 months. The total clinical response rate, clinical cure time, difference in tumor volume reduction, visual analogue scale (VAS) score, postoperative Harris standard evaluation results, recurrence rate, and incidence of adverse drug reactions were compared between the two groups. Results  The total clinical effective rate of the observation group was higher than that of the control group ［90.0%(36/40) vs 70.0%(28/40)］(P=0.025). The clinical cure time of the observation group patients was shorter than that of the control group ［(4.2±1.1)months vs (5.0±1.2)months］, and the difference in tumor volume reduction was greater than that of the control group ［(2.9±0.6)cm3 vs (2.2±0.8)cm3］(all P＜0.05). After treatment, the VAS score of the observation group was lower than that of the control group (P＜0.05). After treatment, the Harris standard evaluation of postoperative breast appearance in the observation group was better than that in the control group (P=0.039). The recurrence rate of GM and the incidence of adverse drug reactions within 6 months of treatment in the observation group were lower than those in the control group (both P＜0.05). Conclusion The combination of Jiawei Xiaoluo pill and ear point pressing beans can significantly improve the clinical symptoms of GM patients, provide overall clinical efficacy, and reduce the risk of recurrence and adverse reactions.
