目的 比较环泊酚和依托咪酯在非体外循环冠状动脉旁路移植术(CABG)中的抗氧化应激作用。方法 选择2021年1—10月在首都医科大学附属北京安贞医院行CABG的患者60例,完全随机分为对照组和观察组,每组30例。对照组在麻醉诱导时使用依托咪酯;观察组在麻醉诱导时使用环泊酚。比较2组麻醉诱导前(T0)以及麻醉诱导后10 min(T1)血流动力学和氧化应激指标的变化。结果 2组患者T1时点心率和平均动脉压(MAP)均低于T0时点,差异均有统计学意义(均P<0.05),而脉搏血氧饱和度(SpO2)组内不同时点比较差异均无统计学意义(均P>0.05)。2组患者心率、MAP、SpO2各时点组间比较差异均无统计学意义(均P>0.05)。2组患者T1时点丙二醛均高于T0时点,超氧化物歧化酶(SOD)均低于T0时点,差异均有统计学意义(均P<0.05);T1时点观察组丙二醛低于对照组,SOD高于对照组[(4.81±0.19)μmol/L比(5.09±0.32)μmol/L、(77.2±4.8)kU/L比(72.2±2.9)kU/L],差异均有统计学意义(t=4.121、4.877,均P<0.001)。结论 在非体外循环CABG中,环泊酚用于麻醉诱导的抗氧化应激作用优于依托咪酯。
Objective To compare the anti-oxidant stress effects of cyclophenol and etomidate in off-pump coronary artery bypass grafting(CABG). Methods From January to October 2021, 60 patients undergoing CABG at Beijing Anzhen Hospital, Capital Medical University were selected. The patients were completely randomized into control group and observation group, with 30 cases in each group. The control group was used etomidate during anesthesia induction, and the observation group was used cyclophenol during anesthesia induction. The changes of hemodynamics and oxidative stress indicators at the time points before anesthesia induction(T0) and 10 minutes after anesthesia induction(T1) were compared in the two groups. Results The heart rate and mean arterial pressure (MAP) at T1 were lower than those at T0 in the two groups (all P<0.05), and there was no statistically significant difference at different time points in the pulse oxygen saturation(SpO2) in the two groups(both P>0.05). There was no statistically significant difference in heart rate, MAP and SpO2 at each time point between the two groups (all P>0.05). Malondialdehyde at time point T1 in the two groups were higher than those at time point T0, and superoxide dismutase (SOD) was lower than that at time point T0 (all P<0.05). Malondialdehyde in the observation group at time point T1 was lower than that in the control group, and SOD was higher than that in the control group[(4.81±0.19)μmol/L vs (5.09±0.32)μmol/L, (77.2±4.8)kU/L vs (72.2±2.9)kU/L](t=4.121, 4.877, both P<0.001). Conclusion Anti-oxidative stress effect of cyclophenol induced by anesthesia is superior to etomidate in off-pump CABG.
