腹腔镜卵巢囊肿剥除术后不留置尿管的安全性及可行性研究

• 摘要：

• 目的  探讨腹腔镜卵巢囊肿剥除术后不留置尿管的安全性及可行性。方法  选取2019年12月31日至2020年12月31日在北京大学人民医院行腹腔镜卵巢囊肿剥除术的120例患者为研究对象，使用随机数字表法将患者分为观察组和对照组，各60例。对照组术后24 h后拔除尿管，观察组在手术结束时立即拔除尿管。比较2组尿潴留发生率、尿管重置率、术后首次自行排尿时间、术后首次自行排尿量、术后首次下床活动时间及术后住院时间。结果  2组尿潴留发生率、尿管重置率比较差异均无统计学意义（均P=1.000）。观察组术后首次自行排尿时间、术后首次下床活动时间均短于对照组［（188±153）min比（245±123）min、（270±222）min比（607±306）min］（t=6.26，P=0.02；t=5.28，P＜0.01）。2组术后首次自行排尿量、术后住院时间比较差异均无统计学意义（均P＞0.05）。结论  腹腔镜卵巢囊肿剥除患者术后不留置尿管安全可行，能促进患者术后康复。

• Objective    To investigate the safety and feasibility of non-indwelling urinary catheter after laparoscopic ovarian cystectomy. Methods  From December 31, 2019 to December 31, 2020, 120 patients underwent laparoscopic ovarian cystectomy in Peking University People′s Hospital were enrolled. Patients were divided into observation group and control group according to the random number table method, with 60 cases in each group. The catheter was removed 24 h after operation in the control group, and the catheter was removed after operation immediately in the observation group. The incidence of urinary retention, the rate of urinary catheter replacement, the time of first spontaneous urination after operation, the volume of first spontaneous urination after operation, the time of first activity out of bed after operation and the length of hospital stay after operation were compared between the two groups. Results  There were no statistically significant differences in the incidence of urinary retention and the rate of urinary catheter replacement between the two groups (both P=1.000). The time of first spontaneous urination after operation and the time of first activity out of bed after operation in the observation group were shorter than those in the control group ［（188±153）min vs （245±123）min,（270±222）min vs （607±306）min］(t=6.26, P=0.02; t=5.28, P＜0.01). There were no statistically significant differences in the volume of first spontaneous urination after operation and the length of hospital stay after operation(both P>0.05). Conclusion  It is safe and feasible to remove the urinary catheter without indwelling in patients after laparoscopic ovarian cystectomy, which can promote postoperative recovery.
