目的 研究黄莪胶囊对慢性非细菌性前列腺炎(CNP)大鼠前列腺组织病理形态的影响,判断其药效作用。方法 选取无特定病原体级健康雄性Wistar大鼠44只,根据随机数字表法分为假手术组(12只)、模型组(12只)、黄莪胶囊低剂量组(10只)和黄莪胶囊高剂量组(10只)。假手术组不造模,分离大鼠皮肤与阴茎间组织后直接缝合皮肤、不结扎缩窄尿道、蒸馏水5 ml/kg灌胃,模型组、黄莪胶囊低剂量组、黄莪胶囊高剂量组采用尿道部分梗阻法制备CNP大鼠模型,分别给予蒸馏水5 ml/kg及质量浓度为0.1 g/ml、0.4 g/ml的黄莪胶囊混悬液5 ml/kg灌胃,4组均为1次/d,连续给药21 d。而后经麻醉称重、解剖收集组织,着重观察比较各组大鼠体质量、前列腺湿质量、前列腺指数、前列腺肿胀率、前列腺肿胀抑制率,并分别进行苏木精-伊红染色,观察前列腺组织病理形态变化。结果 模型组大鼠的前列腺湿质量和前列腺指数均高于假手术组(均P<0.01);黄莪胶囊低、高剂量组大鼠前列腺湿质量、前列腺指数均低于模型组[(1.58±0.10)、(1.43±0.07)g比(1.73±0.07)g,(0.003 90±0.000 25)、(0.003 60±0.000 20)比(0.004 40±0.000 15)]、且黄莪胶囊高剂量组低于黄莪胶囊低剂量组(均P<0.05)。黄莪胶囊低、高剂量组前列腺肿胀率低于模型组、且黄莪胶囊高剂量组低于黄莪胶囊低剂量组(均P<0.05)。黄莪胶囊高剂量组前列腺肿胀抑制率高于黄莪胶囊低剂量组,差异有统计学意义(P<0.05)。连续给药21 d后,与假手术组相比,模型组大鼠前列腺腺泡上皮细胞受损、脱落严重,腺泡腔内无明显分泌物生成,间质内弥漫大量淋巴及单核细胞,血管充血水肿。与模型组大鼠相比,黄莪胶囊低剂量组大鼠前列腺间质内慢性炎症细胞浸润有一定的减轻,部分腺泡上皮增生、分泌旺盛;黄莪胶囊高剂量组大鼠前列腺间质内无明显炎症细胞浸润,腺泡腔扩大,腺体变形,腺上皮皱襞增多,腺腔内分泌物增多。结论 黄莪胶囊以益气活血之功效能有效改善CNP大鼠前列腺组织炎性损伤。
Objective To study the effect of Huang′e capsule on the pathological morphology of prostate tissues in rats with chronic nonbacterial prostatitis(CNP), and to judge its pharmacodynamic effect. Methods Totally 44 specific pathogen free healthy male Wistar rats were selected and divided into control group(12 rats), model group(12 rats), low-dose Huang′e capsule group(10 rats) and high-dose Huang′e capsule group(10 rats) according to the random number table method. The rats in the control group, no modeling, were isolated from the skin and penis tissues, and then directly sutured the skin, constricted the urethra without ligation, and perfused with 5 ml/kg distilled water. The CNP rats model were prepared in the model group, low-dose Huang′e capsule group and high-dose Huang′e capsule group by partial urethral obstruction. The three group were given 5 ml/kg distilled water and 5 ml/kg 0.1 g/ml and 0.4 g/ml Huang′e capsule suspension by gavage respectively. The four groups were given drug administration once a day for 21 d. Then, the tissues were collected through anesthesia, weighing and dissection, and the body mass, prostate wet mass, prostate index, prostate swelling rate and prostate swelling inhibition rate of rats in each group were observed and compared. Hematoxylin eosin staining was performed to observe the pathological changes of prostate tissues. Results The prostate wet mass and prostate index of the model group were higher than those of the control group(both P<0.01); the prostate wet mass and prostate index of low-dose and high-dose Huang′e capsule groups were lower than those of the model group[(1.58±0.10),(1.43±0.07)g vs (1.73±0.07)g;(0.003 90±0.000 25),(0.003 60±0.000 20) vs (0.004 40±0.000 15)], and both of the high-dose Huang′e capsule group were lower than those of low-dose Huang′e capsule group(all P<0.05). The prostate swelling rate of low-dose and high-dose Huang′e capsule groups was lower than that of the model group, and the rate of the high-dose Huang′e capsule group was lower than that of the low-dose Huang′e capsule group(all P<0.05). The prostate swelling inhibition rate of the high-dose Huang′e capsule group was higher than that of the low-dose Huang′e capsule group(P<0.05). After 21 d of continuous administration, compared with the control group, the epithelial cells of prostate acini of the model group were severely damaged and shed, there was no obvious secretion in the acinar cavity, a large number of lymph and monocytes were diffused in the stroma, and the blood vessels were congested and edematous. Compared with the model group, the chronic inflammatory cell infiltration in the prostatic stroma of the low-dose Huang′e capsule group was reduced, and part of the acinar epithelium was hyperplastic and secreted vigorously; in the high-dose Huang′e capsule group, there was no obvious inflammatory cell infiltration in the prostatic stroma, the acinar cavity was enlarged, the glands were deformed, the glandular epithelial folds were increased, and the secretions in the glandular cavity increased. Conclusion Huang′e capsule can effectively improve the inflammatory injury of prostate tissues in CNP rats with the effect of replenishing Qi and activating blood circulation.
