目的 分析体检人群甲状腺结节彩色多普勒超声的疾病特征。方法 收集2018年8月至2021年10月在山东中医药大学附属医院治未病中心参加体检并行甲状腺彩色多普勒超声检查的17 384名体检者的相关资料,分析甲状腺结节在不同年龄及性别人群中的检出情况,并对结节数量、位置、最大直径、甲状腺影像报告和数据系统(TI-RADS)分类等结节特征进行比较。结果 17 384名体检者中,检出甲状腺结节者8 192名,检出率47.12%;男性检出率42.45%(3 866/9 107),女性检出率为52.27%(4 326/8 277);女性甲状腺结节检出率整体高于男性,差异有统计学意义(P<0.001)。甲状腺结节检出率随着年龄的增长而增高(趋势χ2=1 572.454,P<0.001);各年龄段甲状腺结节检出率相比,差异有统计学意义(P<0.001)。8 192名检出甲状腺结节者中,单发结节3 261名(39.81%),多发结节4 931名(60.19%);单侧结节4 702名(57.40%),双侧结节3 490名(42.60%);<10 mm结节6 562名(80.10%),10~<20 mm 结节1 269名(15.49%),≥20 mm结节 361名(4.41%);TI-RADS 2类结节2 092名(25.54%),TI-RADS 3类结节5 577名(68.08%),TI-RADS 4类结节523名(6.38%)。结节数量、位置、最大直径及TI-RADS分类在不同性别间差异均有统计学意义(均P<0.05)。结论 体检人群甲状腺结节检出率较高,并随年龄增长而逐渐升高,女性显著高于男性,以多发、单侧、直径<10 mm、TI-RADS 3类结节为主
Objective To analyze the thyroid nodule characteristics under color Doppler ultrasound in the physical examination population. Methods From August 2018 to October 2021, relevant information of 17 384 examinees who had thyroid color Doppler ultrasound exam in Preventive Treatment Center, the Affiliated Hospital of Shandong University of Traditional Chinese Medicine were collected. The detection of thyroid nodules in different ages and genders was analysis, and the nodule characteristics such as the number, location, size, and thyroid imaging reporting and data system (TI-RADS) classification were compared. Results Among 17 384 examinees, 8 192 cases had thyroid nodules, and the detection rate was 47.12%. The detection rate was 42.45%(3 866/9 107) for men, and 52.27%(4 326/8 277) for women; the overall detection rate of thyroid nodules in women was higher than that in men (P<0.001). The detection rate of thyroid nodules increased with ageing (trend χ2=1 572.454, P<0.001); the detection rate of thyroid nodules was statistically significant among different ages(P<0.001). There were 3 261 cases (39.81%) of single nodule and 4 931 cases (60.19%) of multiple nodules among 8 192 examinees with thyroid nodules. Unilateral nodules were 4 702 cases(57.40%), and bilateral nodules were 3 490 cases (42.60%). There were 6 562 cases (80.10%) with nodule diameter <10 mm, 1 269 cases (15.49%) with 10-<20 mm, and 361 cases (4.41%) with ≥20 mm. TI-RADS class 2 was 2 092 cases (25.54%), TI-RADS class 3 was 5 577 cases (68.08%), and TI-RADS class 4 was 523 cases(6.38%). There were statistical differences in the number, location, maximum diameter, and TI-RADS classification of nodules between genders(all P<0.05). Conclusions The detection rate of thyroid nodules is high in examinees, and gradually increases with ageing. The detection rate in women is significantly higher than that in men. Thyroid nodules are mainly multiple, unilateral, diameter< 10 mm, and TI-RADS class 3.
