温针电针治疗颈源性头痛的效果观察

• 摘要：

• 目的 探讨温针电针治疗颈源性头痛的临床效果。方法 选取首都医科大学附属北京安贞医院疼痛科及山东省胜利油田中心医院疼痛科2020年2月至2021年2月收治的颈源性头痛患者96例。根据随机数字表法将患者分为观察组和对照组，各48例。观察组予温针电针治疗，对照组予内热针治疗。比较2组临床疗效、视觉模拟量表（VAS）评分和Northwick Park颈部疼痛量表（NPQ）评分、血浆炎性细胞因子水平以及治疗相关并发症发生情况。结果 观察组临床疗效明显优于对照组(P=0.030)。治疗后，2组VAS、NPQ评分均明显低于治疗前，且观察组均低于对照组［（2.5±0.4）分比（2.8±0.5）分、（17±5）分比（25±7）分］；2组血浆白细胞介素1（IL-1）、IL-6、IL-8及肿瘤坏死因子α水平均低于治疗前，且观察组均低于对照组（均P＜0.01）。2组治疗相关并发症发生率比较差异无统计学意义（P=1.000）。结论 温针电针治疗颈源性头痛临床效果较好，可有效缓解患者头颈部疼痛，降低炎症细胞因子水平，且安全性好。

• Objective To explore the effects of electro-warm acupuncture on the treatment of cervicogenic headache. Methods From February 2020 to February 2021, 96 patients with cervicogenic headache admitted to Department of Pain Management, Beijing Anzhen Hospital, Capital Medical University and Department of Pain Management, Shengli Oilfield Central Hospital, Shandong Province were enrolled. The patients were divided into observation group and control group according to random number table method, with 48 cases in each group. The observation group was treated with electro-warm acupuncture treatment, and the control group was treated with warm acupuncture treatment. The clinical efficany, visual analog scale (VAS) score, Northwick Park neck pain questionnaire (NPQ) score, levels of plasma inflammatory cytokine and the occurance of adverse reactions related to the treatment were compared between the two groups. Results The clinical efficany of the observation group was significantly better than that of the control group (P=0.030). The scores of VAS and NPQ of the two groups after treatment were lower than those before treatment, and the scores of the observation group were lower than those of the control group ［（2.5±0.4） vs （2.8±0.5）,（17±5） vs （25±7）］; the levels of interleukin-1 (IL-1)， IL-6, IL-8 and tumor necrosis factor-α of the two groups after treatment were lower than those before treatment, and the levels of the observation group were lower than those of the control group (all P<0.01). There was no significant difference in the occurance of adverse reactions related to the treatment between the two groups (P=1.000). Conclusion For patients with cervical headache, the effect of electro-warm acupuncture treatment is obvious, which can effectively relieve neck and head pain, reduce the levels of inflammatory cytokine, with certain safety.
