精细化护理在逆行输尿管软镜碎石取石术围术期的效果分析

• 摘要：

• 目的 探究精细化护理在逆行输尿管软镜碎石取石术围术期的护理效果。方法 选取2019年1月至2020年1月于首都医科大学附属北京安贞医院行逆行输尿管软镜碎石取石术的112例患者，根据随机数字表法分为观察组和对照组，各56例。围术期对照组采用常规护理干预，观察组采用精细化护理干预。比较2组患者护理满意度、肾功能指标水平、手术时间、住院时间、生活质量及并发症发生情况。结果观察组护理满意度高于对照组［98.2%（55/56）比83.9%（47/56）］（P＜0.05）。干预后，2组血尿素氮、血肌酐水平均低于干预前，且观察组均低于对照组［（4.4±1.7）mmol/L比（8.2±1.3）mmol/L、（108±5）μmol/L比（128±7）μmol/L］(均P＜0.001)。观察组手术时间、住院时间均短于对照组，生活质量综合评定量表躯体功能、心理功能、社会功能、物质生活评分均高于对照组(均P＜0.001)。观察组术后并发症发生率低于对照组(P＜0.05)。结论 逆行输尿管软镜碎石取石术围术期实施精细化护理可有效提升患者护理满意度，改善生活质量。

• Objective To explore the perioperative nursing effect of detailed nursing on retrograde soft ureteroscopy lithotripsy. Methods From January 2019 to January 2020, 112 patients who underwent retrograde soft ureteroscopy lithotripsy in Beijing Anzhen Hospital, Capital Medical University were enrolled. Patients were randomly divided into observation group and control group, with 56 cases in each group. During perioperative period, the control group was given routine nursing, and the observation group was given detailed nursing. The nursing satisfaction, renal function index level, operation time, length of stay, quality of life and complications were compared between the two groups. Results   The nursing satisfaction of the observation group was higher than that of the control group ［98.2%（55/56） vs 83.9%（47/56）］（P<0.05）. After intervention, levels of blood urea nitrogen and serum creatinine in both groups were lower than those before intervention, and those of the observation group were lower than those of the control group ［（4.4±1.7）mmol/L vs （8.2±1.3）mmol/L, （108±5）μmol/L vs （128±7）μmol/L］ (all P<0.001). The operation time and length of stay of the observation group were shorter than those of the control group, and scores of somatic function, psychological function, social function and material life of Generic Quality of Life Inventory-74 of the observation group were higher than those of the control group (all P<0.001). The incidence of complications of the observation group was lower than that of the control group (P<0.05). Conclusion  Detailed nursing on retrograde soft ureteroscopy lithotripsy can effectively improve patients′ satisfaction and quality of life during perioperative period.
