目的 设立老年护理门诊,并探讨其实施对患者就诊满意度及护士护理专业自我概念评分的改善效果。方法 制定一系列老年护理门诊服务制度、工作流程及管理条例,明确了老年护理门诊的出诊条件、工作重点及工作内容,开设老年护理门诊,为老年患者进行综合评估,实施个性化慢性病及居家护理指导。选取2021年5—12月在首都医科大学附属北京天坛医院老年护理门诊就诊的老年患者、出诊的老年专科护士、合作的医师为研究对象。对就诊患者进行满意度电话随访调查,对合作医师进行老年护理门诊认可度调查,对出诊护士于出诊前后进行护理专业自我概念量表评价。结果就诊1周后,在172例就诊患者中,共有164例患者接受了电话随访,调查其对老年护理门诊护理服务的满意度,总体非常满意率达97.0%(159/164)。20名合作医师对老年护理门诊认可度为90.0%(18/20)。老年护理门诊开诊后15名护士的护理专业自我概念量表各分项评分及总评分均高于开诊前[总评分:(98.7±5.6)分比(63.0±3.6)分],差异均有统计学意义(均P<0.001)。结论 本院老年护理门诊的开设工作流程顺畅,工作内容能够满足老年患者就诊需求,能够有效保障老年患者就诊满意度,提升出诊老年专科护士职业价值感,得到了合作医师的认可。
Objective To establish a geriatric nursing clinic, and to explore the improvement effect of its implementation on patients′ satisfaction and nurses′ nursing professional self-concept score. Methods The geriatric nursing clinic was established, and a series of service systems, work procedures and management regulations was formulated to clarify the conditions for visits, work priorities and work content of the geriatric nursing clinic. Comprehensive assessment was conducted for elderly patients, and personalized guidance for chronic diseases and home care were implemented. Elderly patients, geriatric nurses and cooperative doctors in the geriatric nursing clinic of Beijing Tiantan Hospital, Capital Medical University from May to December 2021 were selected. A telephone follow-up survey was conducted on the satisfaction of patients, a survey was conducted on the recognition of cooperative doctors in the geriatric nursing clinic, and the nurses were evaluated with the nursing professional self-concept scale before and after clinic. Results One week after the visit, 164 of the 172 patients received telephone follow-up to investigate their satisfaction with the nursing service of the geriatric nursing clinic. The overall very satisfactory rate was 97.0%(159/164). The recognition rate of 20 cooperative doctors to geriatric nursing clinic was 90.0%(18/20). The sub item scores and total scores of nursing professional self-concept scale of 15 nurses after the opening of geriatric nursing clinic were higher than those before opening[total score:(98.7±5.6) vs (63.0±3.6)](all P<0.001). Conclusion The establishment of the geriatric nursing clinic in our hospital is successful, and the work content can meet the needs of elderly patients, effectively guarantee the satisfaction of elderly patients, improve the sense of professional value of geriatric nurses, which is recognized by cooperative doctors.
