目的 探讨氧氟沙星滴耳液治疗急性化脓性中耳炎的效果。方法 选取2018年6月至2021年6月于首都医科大学附属北京安贞医院就诊的240例急性化脓性中耳炎患者。采用随机数字表法分为观察组和对照组,各120例。所有患者根据病原菌及药敏结果进行精准抗感染治疗。对照组采用氯霉素滴耳液滴耳治疗,观察组采用氧氟沙星滴耳液滴耳治疗,均连续治疗2周。比较2组治疗前病原菌分布情况、治疗后病原菌总清除率、耳部疼痛评分、听力改善评分、临床疗效及不良反应发生情况。结果 治疗前,2组感染卡他莫拉球菌、流感嗜血杆菌、化脓性链球菌、肺炎链球菌及金黄色葡萄球菌比例比较,差异均无统计学意义(均P>0.05)。治疗后,观察组卡他莫拉球菌、流感嗜血杆菌、化脓性链球菌、肺炎链球菌、金黄色葡萄球菌清除例数分别为19、15、20、27、35例,对照组分别为22、11、13、25、31例,观察组病原菌总清除率明显高于对照组[96.7%(116/120)比85.0%(102/120)](P<0.05)。治疗后,观察组耳部疼痛评分、听力改善评分及总有效率均明显高于对照组[(2.68±0.38)分比(1.61±0.22)分、(2.80±0.33)分比(1.57±0.14)分、82.5%(99/120)比62.5%(75/120)],不良反应发生率明显低于对照组[0.8%(1/120)比8.3%(10/120)],差异均有统计学意义(均P<0.05)。结论 相较于氯霉素滴耳液,氧氟沙星滴耳液治疗急性化脓性中耳炎的病原菌清除率更高,患者临床症状改善更明显,安全性更高。
Objective To investigate the effect of ofloxacin ear drops on treating acute suppurative otitis media. Methods A total of 240 patients with acute suppurative otitis media admitted to Beijing Anzhen Hospital, Capital Medical University from June 2018 to June 2021 were enrolled. They were randomly divided into observation group and control group, with 120 cases in each group. Accurate anti-infective treatment was carried out for all patients according to results of pathogen and drug sensitivity test. In addition, the control group was treated with chloramphenicol ear drops, and the observation group was treated with ofloxacin ear drops. Both groups were treated for 2 weeks. The pathogen distribution before treatment, total pathogen clearance rate after treatment, ear pain score, hearing improvement score, clinical efficacy and adverse reactions were compared between the two groups. Results Before treatment, there were no significant differences in proportions of Moraxococcus catarrans, Haemophilus influenzae, Streptococcus pyogenes, Streptococcus pneumoniae and Staphylococcus aureus infection between the two groups(all P>0.05). After treatment, Moraxococcus catarrans, Haemophilus influenzae, Streptococcus pyogenes, Streptococcus pneumoniae and Staphylococcus aureus were cleared in 19, 15, 20, 27 and 35 cases in observation group, and were cleared in 22, 11, 13, 25, 31 cases in control group, respectively. The total pathogen clearance rate in observation group was higher than that in control group[96.7%(116/120) vs 85.0%(102/120)](P<0.05). After treatment, ear pain score, hearing improvement score, clinical efficacy in observation group were higher than those in control group [(2.68±0.38) vs (1.61±0.22),(2.80±0.33) vs (1.57±0.14), 82.5%(99/120) vs 62.5%(75/120)], and the rate of adverse reaction in observation group was lower than that in control group[0.8%(1/120) vs 8.3%(10/120)](all P<0.05). Conclusions Compared with chloramphenicol ear drops, ofloxacin ear drops in the treatment of acute sublurative otitis media can promote the pathogen clearance rate, and improve the symptoms of patients with higher safety.
