高尿酸血症(HUA)是高血压众多独立危险因素中的一种。其可通过氧化应激、免疫反应、肾素-血管紧张素-醛固酮系统激活、钠离子重吸收和平滑肌细胞增生等多种机制导致血压升高。降尿酸治疗可降低血压水平,但这种现象只在部分高血压患者中出现,提示高血压患者病因的多样性及HUA导致血压升高机制的复杂性。不同种类的降压药物对血尿酸水平同样存在影响,部分血管紧张素Ⅱ受体拮抗剂及钙通道阻滞剂可降低血清尿酸水平。本文就HUA、HUA对高血压转归的影响及降压药物对血清尿酸的影响进行简要的阐述。
Hyperuricemia(HUA) is one of the independent risk factors for hypertension. It can increase blood pressure through various mechanisms such as oxidative stress, immune response, activation of renin-angiotension-aldosterone system, sodium reabsorption and proliferation of smooth muscle cells. Uric acid lowering therapy can reduce blood pressure, but this phenomenon only appears in a part of the patients with hypertension, suggesting that the etiology of hypertension is diverse and the mechanism of HUA causing hypertension is complex. Different kinds of antihypertensive drugs also have influence on the level of serum uric acid. Some angiotensin Ⅱ receptor blockers and calcium channel blockers can reduce the level of serum uric acid. This paper briefly reviews the effect of HUA on the prognosis of hypertension and the effect of antihypertensive drugs on serum uric acid.
