低频电刺激生物反馈治疗和功能锻炼改善子宫全切术后患者盆底肌收缩力的效果比较

  • 目的 比较低频电刺激生物反馈治疗和功能锻炼(Kegel训练)改善子宫全切术后患者盆底肌收缩力的效果。方法 选择江汉大学附属湖北省第三人民医院2018112月收治的因子宫良性病变行经腹子宫全切术的患者127例。所有患者术后治疗前均进行肌力测定。术后1个月能在医院接受低频电刺激生物反馈治疗的87例患者为电刺激组,10次为1个疗程,其中35例完成了2个疗程的治疗;而无法在医院接受治疗,在家进行Kegel训练的40例患者为功能锻炼组,6周为1个疗程,1个疗程后来医院复查。治疗前后进行盆底肌力评估,比较2组治疗前及治疗1个疗程后的盆底肌肌力,比较电刺激组治疗不同疗程的盆底肌肌力变化。结果 治疗1个疗程后2组盆底肌快速收缩力、持续收缩力均较治疗前增大,且电刺激组大于功能锻炼组[(34±10)μV比(30±6)μV、(27±6)μV比(21±6)μV](均P0.05)。电刺激组治疗2个疗程后的快速收缩力大于治疗1个疗程后(P0.05),持续收缩力与治疗1个疗程后比较差异无统计学意义(P0.05)。结论 子宫全切术后低频电刺激生物反馈治疗和Kegel训练对提高盆底肌收缩力均有效,低频电刺激生物反馈治疗优于Kegel训练,疗程长效果更优。

  • Objective To compare the effect of low-frequency electrical stimulation biofeedback and functional exercise(Kegel training) on pelvic floor muscle contractility after total hysterectomy. Methods Totally 127 patients undergoing transabdominal total hysterectomy for benigh diseases of uterus were enrolled from January to December 2018 in the Third Peoples Hospital of Hubei Province, Jianghan University. After operation, muscle strength was measured in all patients before treatment. The 87 patients who were able to receive low-frequency electrical stimulation biofeedback therapy in the hospital 1 month after the operation were divided into the electrical stimulation group, 10 times as a course of treatment, and 35 of them completed two courses of treatment. However, 40 patients who were unable to receive treatment in the hospital and receiving Kegel training at home were divided into the functional exercise group, 6 weeks as a course of treatment, and a course of treatment was followed up by the hospital. The pelvic floor muscle strength was evaluated before and after treatment. The pelvic floor muscle strength of the two groups before and after one course of treatment was compared, and the changes of pelvic floor muscle strength in the electrical stimulation group were compared after different courses of treatment. Results After one course of treatment, the fast contraction force and continuous contraction fast of pelvic floor muscles in the two groups increased than those before treatment, and the electric stimulation group were larger than those of the functional exercise group[(34±10)μV vs 30±6)μV, 27±6)μV vs 21±6)μV(all P<0.05). The fast contractile force after two courses of treatment in electric stimulation group was greater than that after one course of treatment (P<0.05), while there was no significant difference in continuous contractile force between two couses and one course of treatment(P0.05). Conclusions After total hysterectomy, low-frequency electrical stimulation biofeedback therapy and Kegel training are effective to improve pelvic floor muscle contraction force. Low-frequency electrical stimulation biofeedback therapy is superior to the Kegel training, and long course of treatment is better.
