• 摘要：

• 目的 探讨生脉活血汤加减对不稳定型心绞痛患者血清碱性成纤维细胞生长因子（bFGF）和血小板α-颗粒膜蛋白140（GMP-140）水平的影响。方法选择2019年7月至2020年7月在中国人民解放军总医院第三医学中心接受治疗的不稳定型心绞痛患者156例，按照随机数字表法分为对照组和观察组，各78例。对照组患者给予阿司匹林肠溶片、辛伐他汀、酒石酸美托洛尔治疗，观察组在对照组治疗基础上联合生脉活血汤加减方治疗，2组患者均连续治疗1个月。比较2组治疗前后血清bFGF、GMP-140水平，肱动脉内皮依赖性血管舒张功能(FDM)，心功能，各项中医证候积分，以及治疗6个月后的疗效。结果 治疗前，2组血清bFGF和GMP-140水平比较差异均无统计学意义（均P＞0.05）。治疗后，2组血清bFGF和GMP-140水平均高于治疗前，且观察组高于对照组［(247±32)ng/L比（200±29）ng/L，（13.7±4.1）ng/L比（7.4±2.6）ng/L］（均P＜0.05）。治疗前，2组FDM、心功能和各项中医证候积分比较差异均无统计学意义（均P＞0.05）。治疗后，2组FDM和左心室射血分数均高于治疗前、且观察组高于对照组，左心室舒张末期内径、左心室收缩末期内径以及心痛、胸闷、心悸气短、神疲乏力、形寒肢冷、面色苍白积分均低于治疗前、且观察组低于对照组（均P＜0.05）。治疗6个月后，观察组总有效率高于对照组［98.7%(77/78)比80.8%（63/78）］，差异有统计学意义（P=0.001）。结论 生脉活血汤加减通过调节不稳定型心绞痛患者血清bFGF和GMP-140水平以及FMD，改善患者临床症状及心功能，提高对患者的疗效。

• Objective To study the effects of Shengmai Huoxue decoction on serum levels of basic fibroblast growth factor (bFGF) and platelet α-granule membrane protein-140 (GMP-140) in patients with unstable angina pectoris. Methods From July 2019 to July 2020, 156 patients with unstable angina pectoris admitted to the Third Medical Center of Chinese PLA General Hospital were selected. They were randomly divided into control group and observation group, with 78 cases in each group. The control group was treated with aspirin enteric-coated tablets, simvastatin and metoprolol tartrate, while the observation group was treated with Shengmai Huoxue decoction on the basis of the control group. Both groups were treated for 1 month. The serum levels of bFGF and GMP-140, brachial artery flow mediated dilatation (FDM), cardiac function, traditional Chinese medical(TCM) syndromes scores before and after treatment, and curative efficacy after 6 months of treatment were compared between the two groups. Results Before treatment, there were no significant differences in serum levels of bFGF and GMP-140 between the two groups (both P>0.05). After treatment, the serum levels of bFGF and GMP-140 in the two groups were higher than those before treatment, and those in the observation group were higher than those in the control group ［(247±32)ng/L vs (200±29)ng/L, (13.7±4.1)ng/L vs (7.4±2.6)ng/L］ (all P<0.05). Before treatment, there were no significant differences in FDM, cardiac function and TCM syndromes scores between the two groups (all P>0.05). After treatment, FDM and left ventricular ejection fraction of the two groups were higher than those before treatment, and those of the observation group were higher than those of the control group; the left ventricular end diastolic diameter, left ventricular end systolic diameter, heartache, chest tightness, palpitation and shortness of breath, mental fatigue, cold shape and limbs, pale complexion scores of the two groups were lower than those before treatment, and those of the observation group were lower than those of the control group (all P<0.05). After 6 months of treatment, the total effective rate of the observation group was higher than that of the control group ［98.7%(77/78) vs 80.8%(63/78)］, the difference was statistically significant (P=0.001). Conclusion Shengmai Huoxue decoction can improve the clinical symptoms, cardiac function and curative effect of patients with unstable angina pectoris by regulating the serum bFGF, GMP-140 levels and FMD.
