目的 探究原发性开角型青光眼(POAG)患者血清白细胞介素(IL)-2、IL-6、IL-1β、IL-18水平与视神经损伤的关系。方法 选取2018年2月至2020年2月北京中医药大学东方医院诊治的58例POAG患者为观察组,根据视神经损伤程度分为轻度损伤组(19例)、中度损伤组(19例)和重度损伤组(20例);另选取同期58名健康体检者作为对照组。比较观察组和对照组以及观察组各亚组之间血清IL-2、IL-6、IL-1β、IL-18水平差异。结果 观察组和对照组之间以及观察组各亚组之间性别、年龄、体重指数、收缩压、血糖等一般资料差异均无统计学意义(均P>0.05)。观察组血清IL-2、IL-6水平低于对照组,IL-1β、IL-18水平高于对照组,差异均有统计学意义(均P<0.05)。重度损伤组血清IL-2、IL-6水平低于轻度损伤组和中度损伤组[(246±21)ng/L比(276±12)、(266±19)ng/L,(51±31)ng/L比(79±26)、(76±26)ng/L],IL-1β、IL-18水平高于轻度损伤组和中度损伤组[(98±8)ng/L比(88±7)、(90±7)ng/L,(79±10)ng/L比(62±7)、(69±9)ng/L],差异均有统计学意义(均P<0.05)。结论 POAG患者血清IL-2、IL-6水平随着视神经损伤加重而降低,IL-1β、IL-18水平随着视神经损伤加重而升高,表明细胞免疫和炎症反应参与了POAG患者视神经损伤的发生与发展。
Objective To investigate the relationship between the peripheral blood serum interleukin(IL)-2, IL-6, IL-1β, IL-18 levels and optic nerve injury in patients with primary open angle glaucoma(POAG). Methods From February 2018 to February 2020, 58 POAG patients admitted to Dongfang Hospital, Beijing University of Chinese Medicine were selected as observation group. According to the degree of optic nerve injury, they were divided into mild injury group(19 cases), moderate injury group(19 cases) and severe injury group(20 cases), another 58 healthy subjects in the same period were selected as the control group. The levels of IL-2, IL-6, IL-1 β and IL-18 in peripheral blood between the observation group and control group and the levels among the subgroups of the observation group were compared. Results There were no significant differences in general data of gender, age, body mass index, systolic blood pressure and blood glucose between the observation group and control group, and those among the subgroups of the observation group(all P>0.05). The IL-2, IL-6 levels in the observation group were lower and the IL-1β, IL-18 levels were higher than those in the control group (all P<0.05). The levels of IL-2 and IL-6 in severe injury group were lower than those in mild injury group and moderate injury group[(246±21)ng/L vs (276±12),(266±19)ng/L; (51±31)ng/L vs (79±26), (76±26)ng/L], and the levels of IL-1β and IL-18 in severe injury group were higher than those in mild injury group and moderate injury group[(98±8)ng/L vs (88±7), (90±7)ng/L; (79±10)ng/L vs (62±7),(69±9)ng/L], the differences were statistically significant(all P<0.05). Conclusion The levels of serum IL-2, IL-6 involved in cellular immunity in peripheral blood of patients with POAG are significantly lower than those of normal people, the levels decreased with the aggravation of optic nerve injury, the levels of serum IL-1β, IL-18 involved in cellular inflammation are significantly higher than those of normal people, and the levels increases with the aggravation of optic nerve injury, indicating that cellular immunity and inflammatory reaction are involved in POAG optic nerve injury.
