微小RNA-146a和微小RNA-34a及微小RNA-17-5p与老年患者颈动脉粥样硬化的相关性分析

  • 目的 探究微小RNAmiR-146amiR-34amiR-17-5p与老年患者颈动脉粥样硬化发生的相关性。方法 选取20191月至20201月广东省深圳市人民医院收治的132例老年颈动脉粥样硬化患者作为粥样硬化组,另外选取同期于本院体检的80例无颈动脉粥样硬化的老年人作为无粥样硬化组。采集研究对象清晨空腹外周静脉血,分离出单个核细胞,采用实时荧光定量聚合酶链反应法检测外周血单个核细胞miR-146amiR-34amiR-17-5p表达量,分析三者与颈动脉粥样硬化的关系及对斑块性质的预测价值。结果 粥样硬化组miR-146amiR-34amiR-17-5p表达量均显著高于无粥样硬化组[(1.68±0.38)比(1.02±0.23)、(1.85±0.46)比(1.13±0.24)、(1.59±0.28)比(1.00±0.13)],差异均有统计学意义(均P0.001)。miR-146amiR-34amiR-17-5p表达与颈动脉粥样硬化患者斑块性质相关(均P0.05)。不稳定性斑块颈动脉粥样硬化患者miR-146amiR-34amiR-17-5p表达量高于稳定斑块患者,差异均有统计学意义(均P0.001)。miR-146amiR-34amiR-17-5p三者表达互呈正相关(均P0.05)。miR-146amiR-34amiR-17-5p三项指标联合对颈动脉粥样硬化患者斑块性质的预测价值高于单项指标,差异有统计学意义(P0.05),受试者工作特征曲线下面积为0.892,敏感度为86.39%,特异度为70.12%结论 老年颈动脉粥样硬化患者外周静脉血单个核细胞miR-146amiR-34amiR-17-5p表达异常升高,与患者斑块性质相关,三者呈正相关,可用于此病的诊断和斑块性质的预测。

  • Objective To explore the correlation of microRNA(miR)-146a, miR-34a, miR-17-5p and carotid atherosclerosis in elderly patients. Methods From January 2019 to January 2020, 132 elderly patients with carotid atherosclerosis in Shenzhen Peoples Hospital, Guangdong Province were selected as atherosclerosis group, and 80 elderly people without carotid atherosclerosis were selected as non-atherosclerosis group in the hospital at the same time. All subjects were collected fasting peripheral venous blood in the morning to separate mononuclear cells, and real-time fluorescence quantitative polymerase chain reaction was used to detect the expressions of miR-146a, miR-34a, miR-17-5p in peripheral blood mononuclear cell. The relationship between the three factors, carotid atherosclerosis, and predictive value of them on the plaque properties were analyzed. Results   The  expressions of miR-146a, miR-34a, miR-17-5p in atherosclerosis group were significantly higher than those in non-atherosclerosis group[(1.68±0.38 vs 1.02±0.23, 1.85±0.46 vs 1.13±0.24, 1.59±0.28 vs 1.00±0.13)](all P<0.001). The expressions of miR-146a, miR-34a, miR-17-5p were correlated with the properties of carotid atherosclerotic plaque(all P<0.05). The expressions of miR-146a, miR-34a and miR-17-5p in patients with unstable carotid atherosclerotic plaque were higher than those in patients with stable plaque(all P<0.001). The expressions of miR-146a, miR-34a and miR-17-5p were positively correlated with each other(all P<0.05). The predictive value of miR-146a, miR-34a, miR-17-5p combination was higher than that of single index in plaque nature of carotid atherosclerosis, and the difference was statistically significant(P<0.05); the area under the receiver operating characteristic curve was 0.892, the sensitivity was 86.39%, and the specificity was 70.12%. Conclusions Abnormal expression ascending of miR-146a, miR-34a, miR-17-5p in peripheral venous blood mononuclear cells of elderly patients with carotid atherosclerosis is associated with plaque  properties. It is positive correlation between the three factors, which can be used for diagnosis and prediction of plaque properties.
