目的 探讨新疆维吾尔族妊娠期妇女血清25-羟基维生素D水平与妊娠期糖尿病(GDM)的相关性。方法 选取2017年11月至2018年9月在新疆维吾尔自治区人民医院就诊的维吾尔族妊娠期妇女1 015例,根据口服葡萄糖耐量试验检查结果 分为对照组(血糖正常,914例)和GDM组(101例)。采用化学发光法检测2组受试者血清25-羟基维生素D水平,采用Logistic回归方法 分析血清25-羟基维生素D水平与GDM的相关性。结果 GDM组25-羟基维生素D水平明显低于对照组[(20±5)nmol/L比(25±7)nmol/L],差异有统计学意义(t=2.317、P=0.016)。维生素D缺乏者(243例)相对维生素D正常者(772例)发生GDM的风险增加6.134倍(95%置信区间3.988~9.434),差异有统计学意义(χ2=81.859、P=0.001)。维生素D缺乏的孕妇秋冬季相对春夏季发生GDM的风险增加2.373倍(95%置信区间1.112~5.063),差异有统计学意义(χ2=5.189、P=0.026)。结论血清25-羟基维生素D水平与新疆维吾尔族妊娠期妇女GDM的发生密切相关。
Objective To discuss the correlation between serum 25-hydroxyvitamin D level and gestational diabetes mellitus(GDM) in Xinjiang Uygur pregnant women. Methods Totally 1 015 Uygur pregnant women who visited People′s Hospital of Xingjiang Uygur Autonomous Region from November 2017 to September 2018 were enrolled. According to the results of oral glucose tolerance test, the patients were divided into control group(normal blood glucose, 914 cases) and GDM group(101 cases). The serum 25-hydroxyvitamin D level of all pregnant women were measured by chemiluminescence method, and the difference between the two groups was compared. The correlation between serum 25-hydroxyvitamin D level and gestational diabetes mellitus was analyzed by Logistic regression. Results The level of 25-hydroxyvitamin D in GDM group was significantly lower than that in control group [(20±5)nmol/L vs (25±7)nmol/L, t=2.317, P=0.016]. The risk of GDM in vitamin D deficiency patients was 6.134 times higher than that in normal vitamin D person(95% confidence interval: 3.988-9.434,χ2=81.859,P=0.001). The risk of GDM in pregnant women with vitamin D deficiency in autumn and winter was 2.373 times higher than that in spring and summer(95% confidence interval: 1.112-5.063, χ2=5.189, P=0.026). ConclusionSerum 25-hydroxyvitamin D level is related to the occurrence of GDM in Xinjiang Uygur pregnant women.
