消胀贴穴位贴敷对子宫肌瘤术后患者胃肠道功能恢复的作用

• 摘要：

• 目的 探讨消胀贴穴位贴敷对子宫肌瘤切除术后患者胃肠功能恢复的作用。方法 选取2017年9月至2019年2月于河北省衡水市人民医院行子宫肌瘤切除术后胃肠功能障碍患者90例，应用随机数字表法分为对照组与观察组，各45例。对照组子宫肌瘤切除术后行针刺治疗；观察组在对照组针刺治疗基础上联合消胀贴穴位贴敷治疗，共治疗3 d。比较2组术后首次排气、排便时间，治疗前后症状总评分、腹痛、腹胀及恶心呕吐评分，不同时点血清胃泌素水平以及临床疗效。结果 观察组患者术后首次排气、排便时间明显早于对照组［(21±6)h比（27±7）h、(37±9)h比（49±11）h］，差异均有统计学意义（均P＜0.05）。治疗后2组患者的症状总评分和腹痛、腹胀及恶心呕吐评分均低于治疗前，且观察组低于对照组［(0.95±0.47)分比（1.72±0.53）分、(0.31±0.14)分比（0.59±0.23）分、(0.28±0.11)分比（0.47±0.16）分、(0.32±0.15)分比（0.52±0.21）分］，差异均有统计学意义（均P＜0.05）。术后即刻，2组患者血清胃泌素水平均低于术前（均P＜0.05），但组间比较差异无统计学意义（P>0.05）。治疗24 h后，2组患者血清胃泌素水平均高于术后即刻，且观察组高于对照组［(168±16)ng/L比（152±13）ng/L］，差异均有统计学意义（均P＜0.05）。治疗3 d后观察组总有效率高于对照组［97.8%(44/45)比82.2%（37/45）］，差异有统计学意义（χ2=6.591,P=0.037）。结论子宫肌瘤术后采用针刺联合消胀贴穴位贴敷治疗有助于改善患者临床症状，促进其胃肠功能恢复，提高疗效。

• Objective To investigate the effect of Xiaozhang plaster acupoint application on gastrointestinal function in patients with uterine fibroids after operation. Methods Totally 90 patients with gastrointestinal dysfunction after uterine myomectomy in Hengshui People′s Hospital， Hebei Province, from September 2017 to February 2019, were enrolled. The patients were divided into the control group and the observation group using random number table,with 45 cases in each group. In the control group, acupuncture therapy was performed after resection of uterine fibroids; the observation group was treated with Xiaozhang plaster acupoint application treatment on the basis of acupuncture treatment for 3 days. The time of exhaust and defecation, the total score of symptoms, the score of abdominal pain, abdominal distention, nausea and vomiting, the level of serum gastrin at different time points and clinical effect were compared between the two groups. Results The first exhaustion and defecation time in observation group were significantly earlier than those in control group ［(21±6)h vs (27±7)h, (37±9)h vs (49±11)h，all P＜0.05］. After treatment, the total symptom score and abdominal pain, bloating and nausea and vomiting scores of the both groups were lower than those before treatment, and observation group were lower than control group［(0.95±0.47) vs （1.72±0.53）, (0.31±0.14) vs （0.59±0.23）, (0.28±0.11) vs （0.47±0.16）, (0.32±0.15) vs （0.52±0.21），all P＜0.05］. Immediately after surgery, the serum gastrin levels in the two groups were lower than those before surgery(P＜0.05) but there was no significant difference between the two groups(P>0.05). After 24 h of treatment, the serum gastrin levels in the two groups were higher than those in immediate postoperative; the serum gastrin levels of the observation group was higher than that of the control group［(168±16)ng/L vs (152±13)ng/L， P＜0.05 ］. After 3 days of treatment, the total effective rate of observation group was statistically higher than that of control group ［97.8%(44/45) vs 82.2%(37/45), χ2=6.591, P=0.037］ConclusionAcupuncture combined with augmentation acupoint application after uterine fibroids can help to improve the clinical symptoms;Xiaozhang plaster improves the recovery of gastrointestinal function and therapeutic effect.
