目的探讨木丹颗粒联合羟苯磺酸钙治疗糖尿病周围神经病变(DPN)的临床效果。方法选取2017年11月至2018年11月青岛大学附属医院收治的DPN患者201例为研究对象,按随机数字表法分为对照组(100例,羟苯磺酸钙治疗)和观察组(101例,木丹颗粒+羟苯磺酸钙治疗)。比较2组患者治疗后的临床效果、治疗前后周围神经病变症状评分、腓总神经传导速度、血清高敏C反应蛋白(hs-CRP)、内皮素1及一氧化氮水平,比较治疗期间出现的不良反应。结果观察组总有效率高于对照组[93.1%(94/101)比83.0%(83/100)],差异有统计学意义(P<0.05)。治疗后观察组麻木、感觉减退、烧灼感、疼痛症状评分及总分低于对照组,差异均有统计学意义(均P<0.05)。治疗后观察组腓总神经感觉神经传导速度及运动神经传导速度快于对照组[(45.5±2.2)m/s比(37.8±2.5)m/s、(46.8±2.0)m/s比(40.5±1.7)m/s],差异均有统计学意义(均P<0.05)。治疗后观察组hs-CRP、内皮素1均低于对照组,而一氧化氮高于对照组,差异均有统计学意义(均P<0.05)。2组不良反应发生率差异无统计学意义(P>0.05)。结论木丹颗粒联合羟苯磺酸钙治疗DPN疗效确切,可提高患者腓总神经传导速度,改善临床症状,减轻炎性反应及氧化应激,安全性较高,可为临床治疗提供参考。
ObjectiveTo investigate the clinical effect of Mudan granule combined with calcium dobesilate on diabetic peripheral neuropathy(DPN). MethodsTotally 201 patients with DPN admitted to the Affiliated Hospital of Qingdao University from November 2017 to November 2018 were selected. The patients were randomly divided into control group(100 cases, treating with calcium dobesilate) and observation group(101 cases, treating with mudan granule+calcium dobesilate). The clinical efficacy, peripheral neuropathy symptoms, nerve conduction velocity, serum highsensitivity C reactive protein(hsCRP), endothelin1 and nitric oxide levels were compared between the two groups before and after treatment.The adverse reactions during treatment were compared. ResultsThe total clinical effective rate was higher than that of observation group[93.1%(94/101) vs 83.0%(83/100)](P<0.05). After treatment, the symptoms of numbness, hypoesthesia, burning sensation, pain and total scores of the observation group were lower than those of the control group; the common peroneal nerve sensory nerve conduction velocity and motor nerve conduction velocity in the observation group were faster than those in the control group[(45.5±2.2)m/s vs (37.8±2.5)m/s, (46.8±2.0)m/s vs (40.5±1.7)m/s, all P<0.05]; hsCRP and endothelin1 in the observation group were lower than those in the control group, while nitric oxide in the observation group was higher than that in the control group(all P<0.05). There was no significant difference of adverse reactions between the two groups(P>0.05). ConclusionsMudan granule combined with calcium dobesilate is effective in the treatment of DPN. It can increase the conduction velocity of common peroneal nerve, improve clinical symptoms, alleviate inflammatory reaction and oxidative stress.
