胺碘酮联合厄贝沙坦治疗慢性心力衰竭合并心律失常的临床效果

• 【摘要】目的    探讨胺碘酮联合厄贝沙坦治疗慢性心力衰竭合并心律失常的临床效果。方法    选取2016年12月至2017年12月在河北大学附属医院诊治的慢性心力衰竭合并心律失常患者82例作为研究对象，采用随机数字表法分为对照组和观察组，每组41例。对照组给予常规基础药物治疗，而对观察组患者在基础药物基础上予以胺碘酮联合厄贝沙坦药物治疗。对比2组患者治疗前后心功能指标、治疗后窦性心律维持率及不良反应发生情况。结果    治疗后2组患者的每分输出量、左心室射血分数均较治疗前提高，且观察组高于对照组［（4.70±0.25）L/min比（4.19±0.22）L/min、（50.6±1.1）%比（45.0±0.9）%］；左心室舒张末期内径及收缩末期内径均较治疗前减小，且观察组小于对照组［（55.9±3.0）mm比（60.0±3.1）mm、（51.1±2.2）mm比（55.0±2.6）mm］；差异均有统计学意义（均P＜0.05）。观察组治疗1、3、6个月后窦性心律维持率高于对照组，差异均有统计学意义（均P＜0.05）。2组患者不良反应发生率差异无统计学意义（P＞0.05)。结论    对于慢性心力衰竭合并心律失常患者，采用胺碘酮与厄贝沙坦联合治疗可有效调控患者的心率，提高患者心功能水平。

• 【Abstract】Objective    To investigate the clinical effect of amiodarone combined with irbesartan on chronic heart failure complicated with arrhythmia. Methods    A total of 82 patients with chronic heart failure and arrhythmia were enrolled from December 2016 to December 2017 in Affiliated Hospital of Hebei University. They were randomly divided into control group and observation group, with 41 cases in each group. The control group was treated with routine medication. The observation group was treated with amiodarone and irbesartan on the basis of routine medication. Cardiac function indexes, sinus rhythm maintenance rate after treatment and adverse reactions were analyzed. Results    After treatment, cardiac output per minute and left ventricular ejection fraction were improved and the indexes in the observation group were significantly higher than those in the control group［（4.70±0.25）L/min vs （4.19±0.22）L/min, （50.6±1.1）% vs （45.0±0.9）%］; left ventricular end-diastolic and end-systolic dimensions diminished and the indexes in the observation group were significantly lower than those in the control group［（55.9±3.0）mm vs （60.0±3.1）mm, （51.1±2.2）mm vs （55.0±2.6）mm］(all P＜0.05). Sinus rhythm maintenance rates after 1, 3 and 6 months of treatment in the observation group were significantly higher than those in the control group(all P<0.05). There was no significant difference in the incidence of adverse reactions between groups(P>0.05). Conclusion    Amiodarone combined with irbesartan treating patients with chronic heart failure complicated with arrhythmia is effective in promoting sinus rhythm restoration and improving heat function.
