XperCT在中晚期原发性肝癌患者经导管肝动脉化疗栓塞术中的应用价值

  • 【摘要】目的    探讨XperCT在经导管肝动脉化疗栓塞术(TACE)治疗中晚期原发性肝癌中的应用价值。方法    选取2016年12月至2018年12月江苏省肿瘤医院收治的82例中晚期原发性肝癌患者,按照随机数表法分为观察组与对照组,每组41例。2组均行TACE综合介入治疗,其中观察组在治疗过程中应用XperCT检查,对照组行常规数字减影血管造影检查。比较2组TACE平均治疗次数、治疗血管准确率、碘油沉积效果,每次介入治疗的时间及辐射量。结果    观察组共接受TACE治疗68次,平均(1.7±0.5)次;对照组共接受TACE治疗65次,平均(1.6±0.4)次;2组TACE平均治疗次数的差异无统计学意义(t=0.877,P=0.382)。观察组肿瘤供血动脉共73条,对照组肿瘤供血动脉共72条,观察组治疗血管准确率明显高于对照组[93.2%(68/73)比76.4%(55/72)],差异有统计学意义(χ2=7.913,P=0.005)。观察组碘油沉积Ⅰ型的次数占比为91.2%(62/68),明显高于对照组的70.8%(46/65),差异有统计学意义(χ2=9.067,P=0.003)。观察组介入治疗时间明显长于对照组[(72±9)min比(60±9)min],辐射量明显少于对照组[(38±7)mGy比(55±8)mGy],差异均有统计学意义(t=7.599、13.674,均P<0.001)。结论    XperCT在中晚期原发性肝癌患者TACE治疗中的应用有利于发现肝外供血动脉,提高治疗血管准确率,并改善碘化油沉积情况,且辐射量少。

  • 【Abstract】Objective    To investigate the application value of XperCT in the treatment of advanced primary liver cancer with transcatheter arterial chemoembolization(TACE). Methods    Eighty-two patients with advanced primary liver cancer who were admitted to Jiangsu Cancer Hospital from December 2016 to December 2018 were randomly divided into observation group and control group, with 41 cases in each group. TACE comprehensive interventional therapy was performed in both groups. The observation group was treated with XperCT during intervention, while the control group underwent routine digital subtraction angiography. Average times of TACE, accuracy of target vessel detection, lipiodol deposition, duration time of intervention and radiation quantity were analyzed. Results    The observation group received 68 times of TACE, with an average of (1.7±0.5); the control group received 65 times of TACE, with an average of (1.6±0.4); there was no significant difference of the average treatment times between groups(t=0.877, P=0.382). Seventy-three tumor-supplying arteries were obtained in observation group; 72 tumor-supplying arteries were obtained in control group; the accuracy of target vessel detection in observation group was significantly higher than that in control group[93.2%(68/73) vs 76.4%(55/72)](χ2=7.913, P=0.005). The proportion of iodized oil deposition type Ⅰ was 91.2%(62/68) in observation group, which was significantly higher than that in control group[70.8%(46/65)](χ2=9.067, P=0.003). The duration time of intervention in observation group was significantly longer and radiation quantity was significantly less than those in control group[(72±9)min vs (60±9)min, (38±7)mGy vs (55±8)mGy](t=7.599, 13.674; both P<0.001). Conclusion    Application of XperCT in TACE for advanced liver carcinoma can help display extrahepatic feeding arteries, increase the detection accuracy for target vessel, ameliorate iodized oil deposition and reduce radiation quantity.
