【摘要】目的 观察妇科肿瘤相关性贫血治疗效果与抗贫血治疗前后促红细胞生成素(EPO)水平的相关性。方法 选取2016年1月至2018年1月新疆维吾尔自治区人民医院收治的妇科肿瘤相关性贫血患者200例,根据治疗方式的不同分为放疗组(102例)和化疗组(98例)。分别检测2组患者抗贫血治疗前后的血红蛋白含量、血清铁蛋白含量及EPO水平,并对2组患者EPO变化情况与血红蛋白变化情况进行相关性分析。结果 治疗前2组血红蛋白和血清铁蛋白含量差异无统计学意义(P>0.05),化疗组EPO水平明显高于放疗组[(42.8±4.8)IU/L比(37.2±3.1)IU/L](P<0.01)。治疗后2组血红蛋白含量均较治疗前明显上升,血清铁蛋白含量均较治疗前明显下降,EPO水平均较治疗前明显下降,差异均有统计学意义(均P<0.05);但化疗组EPO水平仍明显高于放疗组[(15.2±2.9)IU/L比(12.2±2.2)IU/L](P<0.01)。Pearson分析显示,放疗组患者的血红蛋白变化与EPO变化呈明显负相关(r=-0.852,P<0.001),化疗组患者的血红蛋白变化与EPO变化无明显相关性(r=0.102,P=0.152)。结论 妇科肿瘤患者体内EPO水平可反映肿瘤相关性贫血的严重程度,其中,EPO水平的变化与放疗患者的抗贫血治疗效果高度相关,而与化疗患者无明显相关性。
【Abstract】Objective To observe the correlation between the therapeutic effect of gynecological cancer-related anemia and the level of erythropoietin(EPO) before and after anti-anemia treatment. Methods A total of 200 patients with gynecological cancer-related anemia admitted to People′s Hospital of Xinjiang Autonomous Region from January 2016 to January 2018 were enrolled. According to different therapeutic methods, they were divided into radiotherapy group(102 cases) and chemotherapy group(98 cases). Levels of hemoglobin(HB), serum ferritin(SF) and EPO were tested before and after anti-anemia treatment. The correlation between changes of EPO and HB was analyzed. Results There was no significant difference of HB and SF contents between groups before treatment(P>0.05); EPO level in chemotherapy group was significantly higher than that in radiotherapy group[(42.8±4.8)IU/L vs (37.2±3.1)IU/L](P<0.01). After treatment, HB level significantly increased, SF and EPO levels significantly decreased in both groups(P<0.05); EPO level in chemotherapy group was still significantly higher than that in radiotherapy group[(15.2±2.9)IU/L vs (12.2±2.2)IU/L](P<0.01). Pearson analysis showed that the change of EPO in radiotherapy group was negatively correlated with HB( r=-0.852, P<0.001); changes of EPO and HB showed no significant correlation in chemotherapy group(r=0.102, P=0.152). Conclusions EPO level in patients with gynecologic cancer can reflect the severity of cancer-related anemia. EPO level is highly correlated with the efficacy of anti-anemia treatment in patients undergoing radiotherapy, but shows no significant correlation in chemotherapy.
