【摘要】目的 探究乳腺影像报告及数据系统(BI-RADS)结合超声造影诊断良恶性乳腺肿块的价值。方法 回顾性分析江苏大学附属张家港澳洋医院2016年8月至2018年8月收治的142例疑似乳腺癌患者,均采用BI-RADS实施分级诊断,再进行超声造影诊断。比较BI-RADS、超声造影及联合诊断的敏感度、特异度、准确率、阳性预测值和阴性预测值。结果 142例患者共有146个肿块,其中良性85个,恶性61个。BI-RADS与超声造影诊断的敏感度、特异度、准确率、阳性预测值及阴性预测值之间差异均无统计学意义(均P>0.05)。联合诊断的特异度、准确率及阳性预测值明显高于BI-RADS[91.8%(78/85)比72.9%(62/85)、91.8%(134/146)比78.8%(115/146)、88.9%(56/63)比69.7%(53/76)],差异均有统计学意义(均P<0.01)。联合诊断的敏感度、特异度、准确率、阳性预测值及阴性预测值与超声造影比较,差异均无统计学意义(均P>0.05)。结论 BI-RADS结合超声造影可有效提高乳腺肿块良恶性诊断准确性,降低误诊率,为临床诊断乳腺肿块提供更为准确的影响学依据。
【Abstract】Objective To explore the value of breast imaging reporting and data system(BI-RADS) combined with contrast-enhanced ultrasound in the differential diagnosis of benign and malignant breast tumors. Methods Totally 142 patients with suspected breast cancer admitted to Zhangjiagang Aoyang Hospital Affiliated to Jiangsu University from August 2016 to August 2018 were retrospectively analyzed. All patients were diagnosed by BI-RADS and contrast-enhanced ultrasound. Sensitivity, specificity, accuracy, positive predictive value and negative predictive value of BI-RADS, contrast-enhanced ultrasound and combined diagnosis were analyzed. Results There were 146 masses including 85 benign masses and 61 malignancies in 142 patients. There were no significant differences of sensitivity, specificity, accuracy, positive predictive value and negative predictive value between BI-RADS and contrast-enhanced ultrasound(all P>0.05). Specificity, accuracy and positive predictive value of combined diagnosis were significantly higher than those of BI-RADS[91.8%(78/85) vs 72.9%(62/85), 91.8%(134/146) vs 78.8%(115/146), 88.9%(56/63) vs 69.7%(53/76)](all P<0.01). There were no significant differences of sensitivity, specificity, accuracy, positive predictive value and negative predictive value between combined diagnosis and contrast-enhanced ultrasound(all P>0.05). Conclusion BI-RADS combined with contrast-enhanced ultrasound can effectively improve diagnostic accuracy and reduce misdiagnosis rate in the differential diagnosis of benign and malignant breast tumors.
