胫骨高位截骨术治疗膝内翻内侧间室骨关节炎的临床效果

• 【摘要】目的    探讨胫骨高位截骨术（HTO）治疗膝内翻内侧间室骨关节炎的临床效果。方法    回顾性分析2016年6月至2018年6月HTO治疗膝内翻内侧间室骨关节炎的38例（47膝）患者的病历资料，病例来源为首都医科大学宣武医院20例（26膝），北京市怀柔区中医医院18例（21膝）。术后配合中药治疗及规范功能锻炼。随访至术后6个月时通过疼痛视觉模拟量表(VAS)，X线片测量胫骨近端内侧角(MPTA)和股胫角，美国西部Ontario和McMaster大学骨关节炎指数(WOMAC)评估手术疗效。结果    所有患者术后未出现切口感染、皮缘坏死及下肢深静脉血栓等并发症。术后6个月随访复查X线片显示截骨处均骨性愈合，VAS评分、MPTA、股胫角、WOMAC评分均较术前均明显改善［（2.4±0.7）分比（7.2±0.9）分、（90.9±0.9）°比（82.2±3.7）°、（170.5±2.0）°比（188.5±8.6）°、（49±10）分比（107±11）分］，差异均有统计学意义（均P＜0.05）。结论    HTO治疗膝内翻内侧间室骨关节炎能够减轻疼痛、改善畸形，临床效果满意。

• 【Abstract】Objective    To analyze the clinical effect of high tibial osteotomy（HTO） on medial compartment knee osteoarthritis. Methods    From June 2016 to June 2018, 38 patients(47 knees) with medial compartment knee osteoarthritis who underwent HTO in Xuanwu Hospital, Capital Medical University（20 cases, 26 knees） and Beijing Huairou Hospital of Traditional Chinese Medicine（18 cases, 21 knees） were retrospectively analyzed. All patients were treated with traditional Chinese medicine and functional exercise after surgery. Visual Analogue Scale(VAS) score, medial proximal tibial angle(MPTA) and femorotibial angle（FTA） measured by X-ray and Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC) were analyzed 6 months after surgery. Results    No incision infection, skin edge necrosis and deep venous thrombosis were observed. Six months after surgery, X-ray showed bony union in all patients; VAS score, MPTA, FTA and WOMAC score were significantly improved compared with those before operation［（2.4±0.7） vs （7.2±0.9）, （90.9±0.9）° vs （82.2±3.7）°, （170.5±2.0）° vs （188.5±8.6）°, （49±10） vs （107±11）］(all P＜0.05). Conclusion    HTO can effectively relieve pain and improve deformity of medial compartment knee osteoarthritis.
