提高集束化预防措施依从性对重症患者呼吸机相关性肺炎发生率的影响

• 【摘要】目的    探讨提高集束化预防措施依从性对重症患者呼吸机相关性肺炎（VAP）发生率的影响。方法    采用前后对照的前瞻性研究方法，选择2016年1月至2017年12月西安交通大学第一附属医院重症医学科（ICU）收治的机械通气患者为研究对象。通过分析2016年VAP集束化预防措施依从性，找出问题，并在2017年通过专人监督检查的方式，提高其集束化预防措施依从性。通过信息化软件统计2年VAP的年发生情况，VAP发生率以例/1 000机械通气日表示。结果    研究期间本院ICU机械通气患者355例，2016年187例，2017年168例，2组患者基本资料比较差异均无统计学意义（均P＞0.05）。与2016年比较，2017年集束化预防措施改进后各项措施的依从性均明显提高，差异均有统计学意义（均P＜0.01）。2016年VAP发生率为8.21例/1 000机械通气日，2017年为3.83例/1 000机械通气日，差异有统计学意义（P=0.040）。结论    提高集束化预防措施依从性可有效降低VAP发生率。

• 【Abstract】Objective    To observe the effect of improving compliance in bundle of preventive measures on the incidence of ventilator-associated pneumonia(VAP) in critical patients. Methods    A prospective study was conducted in critical patients undergoing mechanical ventilation（MV） in intensive care unit（ICU） of the First Affiliated Hospital of Xi′an Jiaotong University from January 2016 to December 2017. Application effect of ventilator bundle of preventive measures in 2016 was reviewed to find out problems. Strategies and countermeasures to improve the compliance of patients were performed in 2017. Annual incidence of VAP was indicated by the VAP cases per 1 000 MV days. Results    A total of 355 ICU patients had MV during 2016 to 2017（187 cases in 2016 and 168 cases in 2017）; the baseline data were similar between patients of 2016 and 2017（P＞0.05）. Compared with that in 2016, the compliance of each measure in ventilator bundle was significantly improved in 2017(P＜0.01). Incidence of VAP was 8.21 cases per 1 000 MV days in 2016 and that droped to 3.83 in 2017, showing a significant difference (P=0.040). Conclusion    Improving the compliance of ventilator bundle can effectively reduce the incidence of VAP.
